If you already have a website but do not have an SSL certificate, you will need to order one from your hosting company. If you are not familiar with SSL terminology, don't worry. It is very simple. All you have to do is place a call to the company currently hosting your website and tell them you need a secure site. The fee is very nominal. Are there any special requirments? Your office needs to be able to access the Internet. Can I print the Patient Information and Medical History for the patient's chart? Yes. Both the Patient Information and Medical History can easily be printed in an easy to read format and placed in their chart. How much does Quick Forms 123 cost? It is a fairly reasonable fee of $199 for the year subscription, which comes out to a little more than $16.50 per month. You have an option to pay monthly for $24.00. If within a two month period you want to upgrade to an annual subscription, we will adjust the rate for you. How do I get started? Go to our website at quickforms123.com and click on Order Now. If you already have a website, you just need to add a link that will take your patient into the secure form section. What if I don't want to change my website, can I still use the form? Yes, you would be able to. Your staff when scheduling appointments would have to give the patient the web address. It would be yourwebaddress.com/form.php What if I don't have a website, can I still take advantage of this software? Yes you can. If you don't have a website, we can create a single page website for you to facilitate the form. It also will give you an opportunity to have your Practice's Name, Address, Phone, Fax, and Office Hours available. What if I don't have a website, and I want more than 1-page?
