  This form requires you have a website that has an SSL certificate. The SSL security is what encrypts the personal data and keeps your patient's information private and secure. Most hosting companies can obtain an SSL certificates for your website at a nominal cost.

Select this option if you want to pay month-to-month. By selecting this option, you will receive a monthly statement of $24.99.  There is a one-time set-up fee for forms that are more than 2-3 page.  Fee depends on complexity of the form and the quantity of pages.
